Austin Performance 2010 Summer Programs Waiver Form
Please print out this page, sign it and return it with payment to complete your registration. If you do not have a printer, please copy the below word for word and sign and date it.  That will suffice as your waiver.

Participant and parent(s)/guardian's) agrees to hold Regent's School of Austin, Austin Performance Volleyball (AP), Performance Camps and Recruiting, LLC (PCR),  any other locations used for or in connection with any Performance summer camp as well as their employees, camp directors, and summer staff harmless from all injuries to persons (both participants and non-participants) occurring as a result of or in any way connected with your presence at AP/PCR camps/clinics, leagues, tournaments, other AP/PCR sponsored events or on the premises used by AP/PCR for these events between the dates of May 1, 2010 and September 1, 2010.  Parent(s)/guardian(s) also authorizes AP/PCR and its staff to act on Parent's behalf according to the best of their knowledge in any emergency situation requiring medical attention. Parent(s)/guardian(s) and participant also understand the inherent risks associated with all sports and physical activity.
